Who Is a Candidate?
Eligibility is determined based on body mass index (BMI) and medical history.
Patients with a personal or family history of thyroid cancer are not candidates for GLP-1 medications. However, alternative medical weight loss options may be available.
Your medical history will be reviewed during your telehealth appointment.
If you are not a candidate, the consultation fee still applies.

Simple Enrollment Process

Step 1 – Complete Enrollment Form
Provide your medical history and basic health information.

Step 2 – Submit Blood Work
If you have completed blood work within the past 12 months, email your results to:

Blood work must be submitted in PDF format and include:
• Name
• Date of testing
• Actual numerical values
​
Results listed only as “normal” will not be accepted. Screenshots are not accepted.
​
If you do not have recent blood work, we will send you a laboratory order for Quest Diagnostics.
​
Required tests include:
• Comprehensive Metabolic Panel
• Lipid Panel
• Thyroid Panel with TSH

Blood work must be reviewed before prescriptions can be submitted. Patients are responsible for any laboratory fees not covered by insurance.

Important Information

If you are not a candidate for Semaglutide or Tirzepatide, alternative treatment options such as Lipo-C are available.
If you decide not to proceed with treatment, or are not a candidate, the $208 program fee is non-refundable.
A home body weight scale is required to track your progress throughout the program.
