PROGRAM INSTRUCTIONS
BEFORE YOUR APPOINTMENT:

Please complete and sign the four electronic documents sent via text message. You have 15 minutes before they expire to complete from time received due to HIPPA regulations.

Complete and sign the credit card authorization sent via text. A credit card is required for this program. We do not accept any other form of payment. Please text a copy of the card holders government issued photo ID as this is required.

If you had blood work completed within the last 12 months please have a copy of the results sent to the following email address: info@suiteskin.com. We do NOT accept screen shots of blood work via text message. They must be emailed in PDF format and must reflect name and date of blood draw.

Please let us know after you have emailed the blood work so we can assure it was not sent to spam. We will review to make sure everything we need is there. If we require additional labs, we will send you a QUEST LAB prescription. It is important your blood work is received. We will not release your prescription without the required blood work. This is for your own safety.

We will send you a prescription to Quest Diagnostics only via text message. You must have your blood work drawn at a Quest diagnostics laboratory. Your blood work will be billed to your health insurance. We are not responsible for any fees incurred in the event your insurance does not cover your blood work. It is your responsibility to inform Quest of your insurance information.

Your program fee of $208 will be processed up to 72 hours prior to your telehealth appointment with our medical team. This is non-refundable.

At your appointment time a member of our medical team will video call you directly. This is not a zoom call. No software or video link is required. The initial consultation typically takes 15 minutes. Your subsequent monthly appointments are approximately 10 minutes.

After your appointment​

After your virtual appointment is completed our pharmacy will contact you directly for medication payment.

You will hear from our pharmacy in 1-2 business days AFTER your telehealth appointment to obtain payment and verify shipping address. You will receive your medication in 3-5 business days after your telehealth appointment. Please call the pharmacy directly if you have not been contacted within 2 business days of your telehealth appointment.

If you receive your medication and the cold packs have melted the medication is still to be used. The ice packs are an added precaution we take since FedEx does not provide temperature controlled transportation vehicles.

If you decide NOT to proceed with the weight loss program, or you are NOT a candidate for your medication of choice, the Program fee payment of $208 is still required and non-refundable. Similarly, how you pay a co-pay at a doctor’s office when they accept insurance, this is our fee. This payment is not based on prescription receipt.

If you are not a candidate for the medication of choice, then you will be offered an alternative medical program.

BOOKING POLICY

IMPORTANT: Future monthly appointments are AUTOMATICALLY booked. If the assigned date and time does not work you must notify us IMMEDIATELY. Your confirmation text message will include date and time of your future appointment. Multiple attempts to cancel/reschedule will result in termination from our program.

CANCELATION POLICY: Cancelation or Rescheduling requests made within 14 days will incur a $75 rebooking fee.

PAYMENT POLICY: Credit Card is the only form of payment accepted. Your credit card payment will be processed up-to 72 hours prior to your appointment.

PICTURE & MEASUREMENT INSTRUCTIONS
Sometimes results are not only shown on the scale!
Providing us with pictures and measurements are a crucial component to monitoring your progress. All pictures and measurements are protected by our HIPPA Policy and are only shared with your team members.
Ideally, measure yourself naked or wearing tight-fitting clothes, in the following areas:

• Neck: Around the center of your neck.
ï‚· • Chest: Around your chest at the nipple line.
ï‚· • Waist: Around your stomach at the belly button line.
ï‚· • Hips: Around the widest part of your hips.
ï‚· • Thighs: Around the largest part of your thigh.
ï‚· • Calves: Around the largest part of each calf.
ï‚· • Arm: Around your upper arm with your arm
resting at your side (under your armpit).

COMMONLY ASKED QUESTIONS

Do I qualify for Semaglutide or Tirzepatide medication? This will be determined by our medical team at the time of your first telehealth appointment and after your full intake has been reviewed.

Is there any tracking number for my FedEx medication? Yes. When our pharmacy calls you for payment you can request the FedEx tracking number.

Is the medication compounded? Yes. It is Semaglutide and B12 combined, or Tirzepatide and B12 combined. Lipotropic Injections are a combination of Methionine, Inositol, Choline, and B12. We also offer an assortment peptides, IV drips and more weight loss options that are distributed by our compound pharmacy.

Is this medication covered by insurance? It can be, but typically it is not. In our experience our patients insurance companies have not covered this medication for weight loss purposes. This is why we use a compound pharmacy and not the brand names “Wegovy,” “Ozempic,” or “Mounjaro.” Additionally, we do not accept insurance, however we can attempt to have your medication covered by your insurance company. You may discuss further with our medical team. Please keep in mind most commercial pharmacies (CVS, Walgreens, etc.) are out of stock.

When should I start my medication? Choose a day and time that you can remain consistent with weekly. Example, if your first dose is on a Wednesday morning, then your next dose should be the following Wednesday morning. For Lipotropic injections, follow this same rule except TWICE per week.

Can I wait to start my medication? Yes. You can start when you’re ready. However please note the medication has a suggested 30 day expiration date. So do not request your medication be shipped until you are ready to start. Additionally, please note if your chosen start date exceeds 60 days past your virtual call with our medical team, you will require a new appointment.

When will I receive my medication? Your medication should be delivered within 3-5 business days from date of your appointment. All medication is shipped overnight by Fedex.

Does my medication delivery require a signature? No. A signature is not required for delivery.

Am I allowed to NOT move forward with the weight loss program? Yes, of course! This is a voluntary program just like any other weight loss program.

If I decide not to move forward with the program after my telehealth appointment, or I am deemed not a candidate, do I get a refund? No. Your $208 program fee still applies. This is time that was spent performing a thorough evaluation during your enrollment process, obtaining necessary signed documentation, and the individual one-on-one time spent with a member of our medical team during your telehealth consultation. Similar to a medical office charging a co-pay, the time our medical team spends with you is fee-based and requires payment.

Is this a compound pharmacy? Yes. Using a compound pharmacy allows us to prescribe our patients weight loss medication at a much more affordable price, compared to using name brand medications at a commercial pharmacy such as Walgreens or CVS.

I had my appointment Saturday, and today is Monday. Why has the pharmacy not called me? Our compound pharmacy requires 1-2 business days for processing. Once they have processed your prescription they will call you for payment and to confirm your shipping address. If you have not received a call from the pharmacy within 1-2 business days, it is your responsibility to contact them directly. Similarly how you would follow up with a commercial pharmacy such as CVS or Walgreens.

Why do I need blood work, and can you send me the medication and I’ll get my blood work drawn after? No. We require blood work to ensure your body can properly metabolize the medication, and ensure baseline wellness. This is for your own safety. We pride ourselves in upholding a high standard of practice and patient care.

Step 1:
Please click HERE to order the necessary syringes on amazon. These will be used to administer your medication at home.

Step 2:
You will be self-administering 1cc of lipotropic medication each time.
Administration Guide:
1. Wipe the top of the vial with alcohol.
2. Inject needle into the vial. Turn vial upside down. Withdraw 1cc of product into the syringe. If
you ordered the syringes we suggested, this means you will fill the entire syringe with product.
3. Remove the syringe from the vial.
4. Wipe the area with alcohol on your stomach or thigh that you plan on injecting with medication.
5. Inject needle into skin at an angle and push entire syringe into skin.
6. Remove needle and discard syringe.
7. Repeat as prescribed.
